Cancellation

To help keep our office running smoothly, and to allow us to provide sufficient time and the best possible care to all clients, we ask that cancellations be made at least 24 hours before your scheduled appointment time. There will be a charge for all appointments not kept or cancelled in less than 24 hours.

  • $25 charge for the first same day cancellation or no show
  • 2nd same day cancellation or no show – fee is 1/2 of scheduled service
  • 3rd same day cancellation or no show – full fee for scheduled service

Appointments scheduled at WholeCare require a 48 hour notice of cancellation; otherwise the full fee for scheduled service will be charged.

Confidentiality

Patient confidentiality is maintained at all times. No information is given to anyone outside of the office unless it is at your request. Unless you request otherwise, information may be shared between Synergy Holistic Health Center practitioners for your benefit.

Financial

Payment is appreciated at the time of service unless alternate arrangements have been made in advance. We accept cash, check, MasterCard, Visa and Discover. At your request, we can provide you with an itemized receipt at the time of service.

If you have insurance benefits for acupuncture and/or massage your co-pay will be collected at the time of service. There is a $10 billing fee for any co-pay not paid at the time of service. There is a $10 billing fee for each monthly billing statement. There is a $10 fee for itemized statements provided after the date of service or end of year statements.

Insurance

It is your responsibility to call the customer service number on your insurance card to inquire about the specific coverage for your plan. All fees not covered by the patient’s insurance plan are the patient’s responsibility. All follow up phone calls for late payments, insurance errors and EOBs are the patient’s responsibility. If your deductible has not been met, payment is due at the time of service. All co-payments are due at the time of service.

Returns

Supplements, herbs & CranioCradles may be returned within 30 days in unopened, non-damaged packaging. A $10 restocking fee will apply to all returns. All sales final for all products other than supplements, herbs & CranioCradles.
